Updated description: A self-assessment handbook that helps CCBHCs and behavioral health organizations see how well they use data. It looks at six areas — the questions you want data to answer, your data itself, your IT and work processes, your technology, your people, and your culture — and rates each one on a five-step scale from basic to advanced. Comes with Excel forms your team can fill out.

Individual Rater Form

An Excel form each team member fills out on their own. For each of 59 practices across the six OACM areas, you pick a score from 1 (basic) to 5 (advanced), note the evidence behind your score, and jot down what it would take to move up a level. When everyone's done, the scores feed into the Team Consolidated Assessment.

Team Consolidated Assessment

An Excel workbook that pulls together scores from up to 20 raters. It flags where people disagreed by two or more points so your team can talk through those spots, records the final agreed-on scores with evidence, and builds a dashboard and one-page summary for leadership. Also includes a short readiness check to see if your organization is set up to do the full assessment.

Sobering centers offer people under the influence of drugs and alcohol a safe alternative to emergency rooms and jails, providing stabilization, health monitoring, case management, and connections to treatment. Although developed in California under CalAIM, the Blueprint for Sobering Care in California: Implementation Guide — available for download below along with a companion financial toolkit — is a practical program design resource for any organization in any state looking to launch or sustain a sobering center, with tools, financial planning guidance, and real-world examples that translate across contexts. Notably, the Commission on Accreditation of Rehabilitation Facilities (CARF) recently announced the nation's first accreditation program for sobering centers, with standards publishing in CARF's 2026 Behavioral Health Standards Manual this January and accreditation available later in 2026.

CCBHC quality reporting depends on data that travels through many hands and systems — from the moment a clinician or front-desk staffer captures it, through storage, extraction, calculation, validation, submission, and ideally back into clinical practice through CQI. The CCBHC Quality Care Metrics: Data Journey Mapping Tool is a practical workbook for any CCBHC — certified or preparing to certify, in any state — to map that full journey for each required clinic-collected measure, pinpoint where data quality breaks down, and turn those findings into a concrete action plan. It includes a measure registry with specs and ownership, lifecycle stage definitions with facilitation questions, a data journey map that distinguishes internal vs. vendor responsibility, a gap analysis and prioritization worksheet, an action plan tied to CQI, and an auto-calculated dashboard — usable before, during, or after a data journey mapping workshop. The tool reflects the March 2025 SAMHSA errata, including the substantive changes to CDF-AD/CDF-CH denominator logic and the Business Days clarification for I-SERV #1 and #2.
